How Do I File for Workers’ Compensation Following a Work-Related Injury or Illness?
You should always notify a supervisor of your workplace injury or illness as quickly as possible—although you have 30 days from the time the injury occurs, or the illness is discovered to do so. You will need to complete and file a WC-14 with the State Board of Workers’ Compensation, sending a copy of the form to your employer and their insurer.
